The MDVIP Wellness Program with Dr. Hermogenes can be paid in quarterly, semi-annual, or annual installments:
QUARTERLY — $432.50 every 3 months
SEMI-ANNUAL — $865.00 every 6 months
ANNUAL — $1,730.00 each year
Note: Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) can be used to cover MDVIP membership fees
